Overview

  • Lead case management operations within a healthcare organization, ensuring exceptional patient care and compliance with standards.
  • Collaborate with multidisciplinary teams to develop and implement effective care plans.
  • Utilize your RN expertise to oversee case management processes and drive quality improvements.
  • Contribute to the strategic direction of the department under the guidance of the Chief Nursing Officer.
  • Ensure compliance with healthcare regulations and accreditation standards.
  • Support professional development and mentorship within the case management team.
  • Participate in organizational initiatives aimed at enhancing patient outcomes and operational efficiency.

Key Responsibilities & Duties

  • Oversee daily operations of the case management department, ensuring efficiency and effectiveness.
  • Develop and implement policies and procedures to optimize patient care delivery.
  • Monitor and evaluate case management outcomes, identifying areas for improvement.
  • Collaborate with healthcare providers to coordinate comprehensive care plans.

Job Requirements

  • Bachelor of Science in Nursing (BSN) required; advanced degree preferred.
  • Current Montana RN license or eligibility for licensure.
  • Minimum of 2 years of experience in hospital case management leadership.
  • Preferred 5 years of experience in hospital case management leadership.
  • Strong knowledge of healthcare regulations and accreditation standards.
  • Proficiency in data analysis and decision-making processes.
  • Excellent communication and collaboration skills.
