Job Description

Overview

  • Provide nursing care and coordinate medical services for individuals in assigned sites.
  • Assess medical needs, plan interventions, and evaluate care outcomes effectively.
  • Perform treatments and special medical care as mandated by protocols.
  • Collaborate with medical providers to ensure comprehensive care management.
  • Monitor and supervise medical care and nutrition issues for individuals.
  • Provide consultation and telephone triage for medical needs 24/7.
  • Administer medications and oversee AMAP certifications for staff.
  • Conduct in-service training on medical issues and maintain updated knowledge.
  • Implement individualized care plans and ensure adherence to safety standards.

Key Responsibilities & Duties

  • Coordinate medical services with residences, schools, and day services.
  • Provide supervision and training to AMAP-certified staff and ensure compliance.
  • Assist with daily living activities, including hygiene, nutrition, and grooming.
  • Attend in-service sessions to stay updated on medical care trends.
  • Maintain accurate records of individual progress and medical interventions.
  • Monitor and address nutrition issues, collaborating with dieticians as needed.
  • Provide medical consultation and triage support to agency staff.
  • Administer medications and ensure proper documentation.
  • Implement care plans tailored to individual medical profiles and needs.

Job Requirements

  • Bachelor of Science in Nursing (BSN) preferred; Associate degree accepted.
  • Updated First Aid and CPR certifications required.
  • Completion of mandated courses in Infection Control and Abuse/Incident Reporting.
  • Minimum of 2-3 years of experience working with adults with developmental disabilities.
  • Strong commitment to safety and adherence to medical care standards.
  • Ability to provide supervision and training to staff effectively.
  • Proficiency in coordinating medical services across various settings.
  • Demonstrated skills in administering medications and medical interventions.
  • Flexibility to work on-site with a commitment to individualized care.
