Overview
- Provide nursing care and coordinate medical services for individuals in assigned sites.
- Assess medical needs, plan interventions, and evaluate care outcomes effectively.
- Perform treatments and special medical care as mandated by protocols.
- Collaborate with medical providers to ensure comprehensive care management.
- Monitor and supervise medical care and nutrition issues for individuals.
- Provide consultation and telephone triage for medical needs 24/7.
- Administer medications and oversee AMAP certifications for staff.
- Conduct in-service training on medical issues and maintain updated knowledge.
- Implement individualized care plans and ensure adherence to safety standards.
Key Responsibilities & Duties
- Coordinate medical services with residences, schools, and day services.
- Provide supervision and training to AMAP-certified staff and ensure compliance.
- Assist with daily living activities, including hygiene, nutrition, and grooming.
- Attend in-service sessions to stay updated on medical care trends.
- Maintain accurate records of individual progress and medical interventions.
- Monitor and address nutrition issues, collaborating with dieticians as needed.
- Provide medical consultation and triage support to agency staff.
- Administer medications and ensure proper documentation.
- Implement care plans tailored to individual medical profiles and needs.
Job Requirements
- Bachelor of Science in Nursing (BSN) preferred; Associate degree accepted.
- Updated First Aid and CPR certifications required.
- Completion of mandated courses in Infection Control and Abuse/Incident Reporting.
- Minimum of 2-3 years of experience working with adults with developmental disabilities.
- Strong commitment to safety and adherence to medical care standards.
- Ability to provide supervision and training to staff effectively.
- Proficiency in coordinating medical services across various settings.
- Demonstrated skills in administering medications and medical interventions.
