Job Description

Overview

  • Provide nursing care and coordinate medical services for individuals in assigned residential sites.
  • Assess medical needs, plan interventions, evaluate outcomes, and perform treatments as mandated.
  • Coordinate care management with other medical providers and monitor individual medical care.
  • Offer medical consultations and telephone triage support to agency staff.
  • Assist with activities of daily living, including food intake, hygiene, and medication administration.
  • Provide AMAP certifications and recertification classes for staff and supervise certified personnel.
  • Coordinate nutritional services with dieticians and assist with menu preparation.
  • Attend in-service training to maintain knowledge of developmental disabilities and medical care.

Key Responsibilities & Duties

  • Perform assessments of individuals’ medical needs and develop care plans accordingly.
  • Coordinate medical services with residences, schools, and day services.
  • Provide supervision and training on medical issues and procedures to staff.
  • Administer medications and ensure compliance with medical protocols.
  • Monitor and supervise AMAP-certified staff and nutrition-related activities.
  • Implement care plans, including positioning, feeding techniques, and ADL skills.
  • Maintain accurate records of individual progress and medical notes.
  • Participate in infection control and abuse/incident reporting training.

Job Requirements

  • Bachelor of Science in Nursing (BSN) preferred; Associate degree accepted.
  • Updated First Aid and CPR certifications are mandatory.
  • Minimum of 2 years of experience working with adults with developmental disabilities.
  • Completion of mandated courses in infection control and abuse/incident reporting.
  • Strong commitment to safety and adherence to medical care standards.
  • Ability to provide medical consultations and training to staff effectively.
  • Proficiency in coordinating care management with other medical providers.
  • Flexibility to work onsite during designated hours.
