Job Description
Overview
- Provide nursing care and coordinate medical services for individuals in assigned residential sites.
- Assess medical needs, plan interventions, evaluate outcomes, and perform treatments as mandated.
- Coordinate care management with other medical providers and monitor individual medical care.
- Offer medical consultations and telephone triage support to agency staff.
- Assist with activities of daily living, including food intake, hygiene, and medication administration.
- Provide AMAP certifications and recertification classes for staff and supervise certified personnel.
- Coordinate nutritional services with dieticians and assist with menu preparation.
- Attend in-service training to maintain knowledge of developmental disabilities and medical care.
Key Responsibilities & Duties
- Perform assessments of individuals’ medical needs and develop care plans accordingly.
- Coordinate medical services with residences, schools, and day services.
- Provide supervision and training on medical issues and procedures to staff.
- Administer medications and ensure compliance with medical protocols.
- Monitor and supervise AMAP-certified staff and nutrition-related activities.
- Implement care plans, including positioning, feeding techniques, and ADL skills.
- Maintain accurate records of individual progress and medical notes.
- Participate in infection control and abuse/incident reporting training.
Job Requirements
- Bachelor of Science in Nursing (BSN) preferred; Associate degree accepted.
- Updated First Aid and CPR certifications are mandatory.
- Minimum of 2 years of experience working with adults with developmental disabilities.
- Completion of mandated courses in infection control and abuse/incident reporting.
- Strong commitment to safety and adherence to medical care standards.
- Ability to provide medical consultations and training to staff effectively.
- Proficiency in coordinating care management with other medical providers.
- Flexibility to work onsite during designated hours.
