Key Responsibilities & Duties

  • Provide care for patients with acute and chronic conditions, ensuring accountability for health outcomes.
  • Lead a care team to deliver coordinated and comprehensive patient care.
  • Engage with hospitalists and specialists to manage patient care transitions effectively.
  • Conduct patient assessments, diagnoses, treatments, and health education.
  • Maintain accurate and concise documentation using advanced technology systems.

Job Requirements

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
  • Board certification in Internal Medicine, Family Medicine, or Geriatrics mandatory.
  • Active state medical license and DEA registration for controlled substances required.
  • Basic Life Support (BLS) certification from AMA or American Red Cross within 90 days of employment.
  • Proficiency in Microsoft Office Suite and healthcare documentation systems.
  • Strong commitment to patient-centered care and mission-driven healthcare delivery.
  • Ability to travel locally for patient care and nationally for training purposes.
