Key Responsibilities & Duties
- Conduct comprehensive assessments to determine patient healthcare needs and establish individualized care plans.
- Administer medications, treatments, and procedures as prescribed by physicians.
- Perform specialized clinical tasks, including wound care, ostomy management, and G-tube care.
- Monitor patient conditions, documenting progress and ensuring adherence to care plans.
- Coordinate care with interdisciplinary teams to provide seamless healthcare delivery.
- Supervise home health aides, offering guidance and conducting supervisory visits.
- Educate patients and families on healthcare practices to promote wellness and independence.
- Respond promptly to changes in patient conditions, ensuring effective communication with care teams.
Job Requirements
- Active Registered Nurse (RN) license in New Jersey, meeting state requirements.
- Bachelor of Science in Nursing (BSN) preferred; Associate’s Degree in Nursing required.
- AHA Basic Life Support (BLS) certification for healthcare providers.
- Minimum of 1 year RN experience, with preference for 3 years in relevant fields.
- Proficiency in clinical skills such as wound care, ostomy management, and G-tube care.
- Experience in home care, hospice, or medical-surgical settings is highly desirable.
- Physical ability to lift, push, or pull up to 50 lbs and stand for extended periods.
- Strong clinical judgment, documentation skills, and time management capabilities.
