Job Description

Overview

  • Coordinate patient care services and facilitate medical case management for primary care patients in a dynamic healthcare environment.
  • Screen patients for program eligibility and assist with insurance verification and enrollment processes.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and referral services.
  • Document clinical interventions and maintain accurate medical records in compliance with HIPAA regulations.
  • Support advocacy efforts to address homelessness and AIDS through effective patient care strategies.
  • Provide professional scheduling and registration services to enhance patient experience and operational efficiency.
  • Participate in quality improvement initiatives and interdisciplinary meetings to optimize healthcare delivery.
  • Assist with paperwork and documentation required for patient services, including transportation and home health aide forms.
  • Perform outreach activities and collaborate with ancillary programs to support patient progress and referrals.

Key Responsibilities & Duties

  • Coordinate intake screenings and identify eligibility for various healthcare programs and services.
  • Collaborate with intake specialists to complete assessments and referrals for primary care patients.
  • Ensure timely documentation of hospitalizations and schedule discharge appointments as needed.
  • Participate in morning huddles and medical staff meetings to discuss patient needs and care plans.
  • Assist medical providers with paperwork and documentation for patient services and entitlements.
  • Provide coverage for intake specialists and support case management programs as required.
  • Manage medical records releases and ensure compliance with turnaround times and regulations.
  • Coordinate with care management teams for subspecialty referrals and interdisciplinary collaboration.
  • Support advocacy efforts and participate in initiatives to improve healthcare outcomes for underserved populations.

Job Requirements

  • High School Diploma or GED required; proficiency in Spanish preferred.
  • Minimum of 2 years of experience in case management and patient care coordination.
  • Strong knowledge of HIV/AIDS, entitlements, and psychosocial assessments.
  • Ability to discern medical emergencies and alert appropriate personnel promptly.
  • Familiarity with HIPAA regulations and electronic medical records systems.
  • Excellent organizational and clinical skills; ability to work evenings and weekends.
  • Experience with interdisciplinary collaboration and quality improvement projects.
  • Commitment to advocacy efforts addressing homelessness and AIDS.
