Overview

- Participate in quality improvement initiatives and interdisciplinary meetings to optimize healthcare delivery.
- Assist with paperwork and documentation required for patient services, including transportation and home health aide forms.
- Perform outreach activities and collaborate with ancillary programs to support patient progress and referrals.
Key Responsibilities & Duties
- Coordinate intake screenings and identify eligibility for various healthcare programs and services.
- Collaborate with intake specialists to complete assessments and referrals for primary care patients.
- Ensure timely documentation of hospitalizations and schedule discharge appointments as needed.
- Participate in morning huddles and medical staff meetings to discuss patient needs and care plans.

- Provide coverage for intake specialists and support case management programs as required.
- Manage medical records releases and ensure compliance with turnaround times and regulations.
- Coordinate with care management teams for subspecialty referrals and interdisciplinary collaboration.

Job Requirements
- High School Diploma or GED required; proficiency in Spanish preferred.
- Minimum of 2 years of experience in case management and patient care coordination.
- Strong knowledge of HIV/AIDS, entitlements, and psychosocial assessments.
- Ability to discern medical emergencies and alert appropriate personnel promptly.
- Familiarity with HIPAA regulations and electronic medical records systems.
- Excellent organizational and clinical skills; ability to work evenings and weekends.
