Key Responsibilities & Duties

• Provide day-to-day leadership and mentoring to Nurse Practitioners and Physician Assistants.
• Carry a focused clinical caseload calibrated to team size and capacity.
• Deliver palliative care consultations and follow-ups across hospital settings.
• Lead goals-of-care and advance care planning conversations with patients and families.
• Promote evidence-based standards and quality documentation practices.
• Support onboarding, shadowing, and quality review processes for team members.
• Build referral relationships and represent the client with hospital partners.

Job Requirements

• Master of Science in Nursing with active New Jersey NP or PA license.
• Minimum of 5 years of palliative care experience required.
• Preferred certification in palliative care (e.g., ACHPN / CHPN).
• 1–2+ years of leadership, mentoring, or supervisory experience.
• Strong communication, clinical judgment, and interdisciplinary collaboration skills.
• DEA + CDS registration or ability to obtain by start date.
• Commitment to delivering patient-centered, evidence-based care.
