Overview
- Lead and manage a team of Advanced Practice Providers in outpatient palliative care settings.
- Provide direct patient care calibrated to team size, ensuring quality and consistency.
- Shape clinical standards and mentor team members to enhance performance and patient outcomes.
- Engage in program-building activities to strengthen palliative care services across facilities.
- Collaborate with interdisciplinary teams to deliver holistic, patient-centered care.
- Support transition planning and care coordination for patients in skilled nursing facilities.
- Participate in quality improvement initiatives and documentation standardization.

Key Responsibilities & Duties
- Mentor and provide clinical support to Nurse Practitioners and Physician Assistants.
- Deliver palliative care consultations and follow-ups in skilled nursing settings.
- Lead goals-of-care discussions and advance care planning with patients and families.
- Promote evidence-based practices and ensure consistent documentation quality.
- Support onboarding, shadowing, and case review workflows for team members.
- Build and maintain referral relationships with community organizations and facilities.
- Coordinate local and regional coverage planning with mileage reimbursement.
- Engage in leadership activities to strengthen team productivity and quality.
Job Requirements
- Master of Science degree with active New Jersey NP or PA license.
- National board certification and DEA + CDS registration or eligibility.
- Minimum 5 years of relevant experience in palliative care or related fields.
- Preferred certification in palliative care (e.g., ACHPN / CHPN).
- Experience in leadership, mentoring, or supervisory roles is advantageous.
- Strong communication, clinical judgment, and interdisciplinary collaboration skills.
- Ability to manage clinical and operational aspects effectively.
- Commitment to quality improvement and patient-centered care.
