Overview

  • Oversee utilization review, discharge planning, and transitions of care to optimize patient flow and resource utilization.
  • Collaborate with interdisciplinary teams to enhance care quality and streamline reimbursement processes.
  • Provide clinical leadership and operational oversight, supervising staff and driving quality improvement initiatives.
  • Serve as a liaison for providers, staff, payers, and community partners to ensure seamless care delivery.

Key Responsibilities & Duties

  • Manage daily operations of case management, including utilization review and discharge planning.
  • Supervise and mentor case management staff, fostering professional growth and development.
  • Collaborate with interdisciplinary teams to ensure high-quality, patient-centered care.
  • Drive quality improvement initiatives to enhance patient outcomes and operational efficiency.
  • Ensure compliance with regulatory standards and optimize reimbursement processes.
  • Act as a clinical resource for staff, providers, and community partners.
  • Facilitate safe and efficient care transitions across the continuum of care.
  • Utilize EPIC systems for effective case management and documentation.

Job Requirements

  • Bachelor of Science in Nursing (BSN) required; Master's in Nursing (MSN) preferred.
  • Unrestricted RN license in Colorado or ability to obtain before start date.
  • Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications required.
  • Minimum of 2 years' experience in case management or utilization review roles.
  • Preferred 4 years' experience in nursing leadership and familiarity with EPIC systems.
  • Bilingual in Spanish and Pediatric Advanced Life Support (PALS) certification preferred.
  • Strong knowledge of regulatory standards and quality improvement practices.
  • Exceptional communication and collaboration skills to work with diverse teams.
