Job Description

Overview

  • Deliver compassionate end-of-life care, ensuring dignity and comfort for patients and their families during critical transitions.
  • Conduct comprehensive assessments to develop personalized care plans addressing physical, emotional, and social needs.
  • Collaborate with interdisciplinary teams to provide holistic and patient-centered care.
  • Educate patients and families about hospice services to foster understanding and trust.
  • Supervise healthcare staff to ensure high standards of care delivery and compliance with regulations.
  • Adapt care plans to accommodate evolving patient conditions and physician directives.
  • Participate in quality improvement initiatives to enhance service delivery and patient satisfaction.
  • Maintain accurate and detailed documentation to ensure effective communication and regulatory compliance.

Key Responsibilities & Duties

  • Perform thorough assessments of patients' conditions to inform and refine care strategies.
  • Develop and implement individualized nursing care plans tailored to patient needs.
  • Supervise and evaluate healthcare staff to ensure quality care delivery.
  • Coordinate with physicians to update care plans as patient conditions evolve.
  • Provide hands-on care and emotional support to patients and their families.
  • Educate families on hospice care processes, resources, and coping strategies.
  • Ensure compliance with regulatory standards and organizational policies.

Job Requirements

  • Associate of Science (AS) in Nursing or equivalent education required.
  • Active Registered Nurse (RN) license with a minimum of 1 year of clinical experience.
  • Preferred experience in hospice, home care, or acute care settings.
  • Exceptional communication and interpersonal skills to engage patients and families effectively.
  • Ability to work collaboratively within an interdisciplinary team environment.
  • Proficiency in clinical data input and maintaining accurate documentation.
