Overview
- Support Medicaid members by connecting them to social and healthcare resources in a community-based, non-clinical role.
- Conduct screenings and interviews to identify health-related social needs and coordinate referrals.
- Document all activities accurately in required systems, ensuring compliance with organizational standards.
- Work in a hybrid schedule, combining onsite, field, and remote work environments.
- Provide outreach and follow-up to ensure services are received and barriers to care are resolved.
- Collaborate with diverse populations, demonstrating cultural sensitivity and adaptability.
- Contribute to the Social Care Network under the 1115 Medicaid Waiver program.
- Participate in training and development to enhance skills and knowledge for effective service delivery.
Key Responsibilities & Duties
- Conduct screenings and interviews with Medicaid members to verify eligibility and obtain consent.
- Connect members with community resources and social services to address health-related social needs.
- Complete and manage referrals using Unite Us and other web-based systems.
- Develop care plans and perform follow-ups to ensure services are received.
- Monitor referral statuses and resolve barriers to care for members.
- Perform outreach activities through phone, virtual meetings, or in-person visits.
- Document all interactions and activities accurately, meeting productivity expectations.
- Collaborate with team members and community partners to optimize service delivery.
Job Requirements
- High School Diploma or GED required; relevant experience preferred.
- 1-2 years of experience in community health, patient navigation, or care coordination.
- Strong computer skills, including proficiency in Microsoft Office and web-based systems.
- Experience with Medicaid populations and Unite Us software is a plus.
- Bilingual candidates and those with a valid driver’s license are strongly preferred.
- Excellent communication, organizational, and time management skills.
- Ability to work independently in both office and community settings.
- Passion for helping underserved communities and culturally sensitive service delivery.
