Overview
- Act as a clinical liaison, connecting patients and families with hospice care services and resources.
- Collaborate with healthcare providers to promote awareness and understanding of hospice care options.

- Participate in a flexible schedule, including four 10-hour shifts and weekend availability.

Key Responsibilities & Duties
- Develop and maintain strong relationships with medical professionals and referral sources.
- Educate patients, families, and healthcare providers about hospice services and benefits.
- Conduct patient assessments to determine eligibility for hospice care based on clinical criteria.
- Coordinate with clinical teams to ensure seamless transitions into hospice care.
- Promote hospice services through community outreach and engagement activities.
- Meet referral and census growth targets to support organizational goals.
- Maintain compliance with all hospice policies, procedures, and regulatory requirements.
Job Requirements
- Bachelor of Science in Nursing (BSN) required, with an active RN license.
- Minimum of one year of experience in healthcare sales, marketing, or related roles.
- Preferred experience in hospice or palliative care environments.
- Strong interpersonal and communication skills to build meaningful connections.
- Reliable transportation, valid driver’s license, and auto insurance required.
- Ability to work in a hybrid model, including weekend shifts as needed.
