Job Description

Overview

  • Provide in-home rehabilitation services through comprehensive evaluation and goal-oriented treatment plans.
  • Ensure patient-centered care in alignment with physician orders and regulatory standards.
  • Collaborate with interdisciplinary teams to optimize patient outcomes and care coordination.
  • Educate patients and caregivers on functional independence strategies and home programs.
  • Maintain accurate and timely documentation in electronic medical records (EMR).
  • Monitor patient progress and adjust treatment plans as clinically indicated.
  • Participate in discharge planning to ensure continuity of care.
  • Adhere to organizational policies and procedures to deliver safe and effective care.

Key Responsibilities & Duties

  • Conduct holistic patient evaluations in home settings to assess rehabilitation needs.
  • Develop individualized treatment plans with measurable short- and long-term goals.
  • Provide skilled occupational therapy services as per physician’s plan of care.
  • Document evaluations, progress notes, reassessments, and discharge summaries accurately.
  • Ensure timely and complete documentation supporting ongoing skilled care.
  • Collaborate with physicians and care team members to coordinate referrals and care plans.
  • Educate patients and caregivers on functional independence strategies and home programs.
  • Respond promptly to clinical communications and secure messages.

Job Requirements

  • Bachelor’s Degree in Occupational Therapy; Master’s Degree preferred.
  • Minimum of 1 year experience in Occupational Therapy; 3 years preferred.
  • Active Occupational Therapist License in New Jersey.
  • AHA Basic Life Support (BLS) Certification required.
  • Proficiency in electronic medical records (EMR) systems.
  • Ability to lift, push, and pull up to 50 lbs and perform clinical duties for up to 8 hours.
  • Experience in home care settings is highly desirable.
  • Strong communication and collaboration skills with interdisciplinary teams.
