Overview
- Provide in-home rehabilitation services through comprehensive evaluation and goal-oriented treatment plans.
- Ensure patient-centered care in alignment with physician orders and regulatory standards.
- Collaborate with interdisciplinary teams to optimize patient outcomes and care coordination.
- Educate patients and caregivers on functional independence strategies and home programs.
- Maintain accurate and timely documentation in electronic medical records (EMR).
- Monitor patient progress and adjust treatment plans as clinically indicated.
- Participate in discharge planning to ensure continuity of care.
- Adhere to organizational policies and procedures to deliver safe and effective care.
Key Responsibilities & Duties
- Conduct holistic patient evaluations in home settings to assess rehabilitation needs.
- Develop individualized treatment plans with measurable short- and long-term goals.
- Provide skilled occupational therapy services as per physician’s plan of care.
- Document evaluations, progress notes, reassessments, and discharge summaries accurately.
- Ensure timely and complete documentation supporting ongoing skilled care.
- Collaborate with physicians and care team members to coordinate referrals and care plans.
- Educate patients and caregivers on functional independence strategies and home programs.
- Respond promptly to clinical communications and secure messages.
Job Requirements
- Bachelor’s Degree in Occupational Therapy; Master’s Degree preferred.
- Minimum of 1 year experience in Occupational Therapy; 3 years preferred.
- Active Occupational Therapist License in New Jersey.
- AHA Basic Life Support (BLS) Certification required.
- Proficiency in electronic medical records (EMR) systems.
- Ability to lift, push, and pull up to 50 lbs and perform clinical duties for up to 8 hours.
- Experience in home care settings is highly desirable.
- Strong communication and collaboration skills with interdisciplinary teams.
