Job Description

Overview

  • Provide professional rehabilitation services to patients in their homes, focusing on functional independence and quality of life improvement.
  • Conduct comprehensive evaluations and develop individualized treatment plans with measurable goals tailored to patient needs.
  • Ensure compliance with physician orders, organizational policies, and regulatory standards in therapy delivery.
  • Collaborate with interdisciplinary care teams to coordinate services and ensure optimal patient outcomes.
  • Educate patients and caregivers on home exercise programs and functional training strategies.
  • Maintain accurate and timely documentation in electronic medical records (EMR) systems.
  • Participate in discharge planning and adjust interventions based on clinical outcomes.
  • Adhere to productivity standards in a part-time home care setting.

Key Responsibilities & Duties

  • Conduct in-home patient evaluations within required timeframes to establish therapy needs.
  • Develop functional treatment plans with both short- and long-term goals.
  • Deliver skilled occupational therapy services as per physician’s plan of care.
  • Maintain precise and complete EMR documentation including evaluations and progress notes.
  • Monitor patient progress and adjust therapy interventions accordingly.
  • Educate patients and caregivers on effective home exercise programs.
  • Collaborate with care team members to ensure coordinated service delivery.
  • Participate in discharge planning and ensure seamless transition of care.
  • Respond promptly to clinical communications and maintain scheduling requirements.

Job Requirements

  • Bachelor’s Degree in Occupational Therapy; Master’s Degree preferred.
  • Minimum of 1 year of experience in occupational therapy; 3 years preferred.
  • Active Occupational Therapist License in New Jersey.
  • AHA Basic Life Support (BLS) Certification required.
  • Proficiency in computer systems and electronic medical records (EMR).
  • Experience in home care therapy is highly desirable.
  • Physical ability to lift, push, and pull up to 50 lbs.
  • Capability to stand and perform clinical duties for extended periods.
