Job Description
Overview
- Serve as a Wellness Nurse in a hybrid work environment, promoting healthier lifestyles and managing acute and chronic conditions.
- Conduct health screenings, monitor progress, and educate members on wellness practices and disease prevention.
- Administer tests and vaccinations, including seasonal immunizations, and participate in health fairs and outreach events.
- Educate members through various channels, including in-person, virtual, and telephonic consultations.
- Collaborate with physicians and specialists to address member health concerns and provide tailored advice.
- Support members by counseling on health benefits and connecting them to internal and community resources.
- Contribute to statistical analysis by recording clinical data and performing additional duties as assigned.
Key Responsibilities & Duties

- Perform additional duties and projects as assigned by management.
Job Requirements
- Valid New York State Registered Nurse (RN) license; Bachelor’s degree in Nursing preferred.
- Minimum two years of experience in ambulatory care, community health, or medical/surgical settings.
- Proficiency in clinical assessment, patient education, motivational interviewing, and critical thinking.
- Intermediate computer literacy and familiarity with Microsoft Outlook, Word, and Excel.
- Bilingual in Spanish preferred; excellent oral and written communication skills required.
- Ability to work flexible hours, including evenings and weekends, as needed.
- Valid NYS Driver’s license preferred for travel within the New York Metropolitan area.
