Overview

  • Lead memory care initiatives, ensuring high-quality services for residents with cognitive challenges.
  • Collaborate with multidisciplinary teams to develop individualized care plans tailored to residents' needs.
  • Provide oversight and training to staff on best practices in memory care.

Key Responsibilities & Duties

  • Develop and oversee memory care programs, ensuring alignment with residents' needs.
  • Provide direct support and guidance to staff in implementing care strategies.
  • Conduct assessments to identify residents' cognitive and emotional requirements.
  • Facilitate training sessions for staff on dementia care techniques and approaches.

Job Requirements

  • Master of Social Work degree required; LMSW or LCSW certification preferred.
  • Minimum of 2 years of experience in memory care or geriatrics; 4 years preferred.
  • Strong knowledge of dementia care practices and cognitive health management.
  • Proficiency in developing and implementing care plans for residents.
  • Excellent communication and interpersonal skills for team collaboration.
  • Ability to lead and train staff on memory care best practices.
