Job Description

Overview

  • Provide case management services to clients with complex medical needs in a community-based housing program.
  • Coordinate healthcare services, referrals, and appointments to ensure clients' health and housing stability.
  • Conduct home visits and collaborate with interdisciplinary teams to assess and address client needs.
  • Serve as a key resource for clients, connecting them with healthcare providers and community resources.
  • Maintain accurate documentation and monitor client health status for effective care planning.
  • Work in a flexible schedule with office days and field visits within the Bronx community.
  • Contribute to a supportive environment that promotes independent living and client well-being.
  • Utilize provided resources, including MetroCard for community travel, to fulfill job responsibilities.

Key Responsibilities & Duties

  • Manage a caseload of clients, coordinating medical appointments and supportive services.
  • Conduct routine home visits to assess client needs and provide ongoing support.
  • Collaborate with healthcare professionals to ensure continuity of care.
  • Monitor clients' health status and assist with care planning and implementation.
  • Maintain accurate and timely documentation of client interactions and progress.
  • Coordinate with interdisciplinary teams to address client health and housing needs.
  • Utilize organizational resources effectively to support client care and program goals.
  • Participate in training opportunities for professional development and skill enhancement.

Job Requirements

  • Active New York State Licensed Practical Nurse (LPN) license.
  • Associate of Applied Science (AAS) degree in Nursing or related field.
  • Minimum of 1 year of experience; 3 years preferred in case management or community health.
  • Strong communication and organizational skills for effective client interaction and documentation.
  • Comfortable traveling throughout the Bronx for home visits and community engagement.
  • Experience with medically complex populations or behavioral health is advantageous.
