Hospice Social Worker Key Responsibilities & Duties

• Provide psychosocial support and social work services to hospice patients and families in various care settings.
• Develop and implement social work care plans, addressing patient and family needs effectively.
• Act as a liaison between hospice staff and inpatient or nursing home personnel.
• Participate in interdisciplinary team meetings to ensure holistic patient care.

Hospice Social Worker Job Requirements

• Master of Social Work degree from an accredited institution is required.
• Active LMSW or LCSW licensure in the state of New York.
• Minimum of one year of experience in a healthcare social work setting.
• Proficiency in Microsoft Word and Excel for documentation and reporting.
• Strong organizational, communication, and interpersonal skills to work within interdisciplinary teams.
• Ability to manage field-based responsibilities, including travel within the Bronx area.
