Job Description

Registered Nurse Overview

  • The Registered Nurse will provide essential healthcare services within a behavioral health framework, supporting clients in community and clinic settings.
  • Conduct initial and ongoing nursing assessments, ensuring comprehensive care plans tailored to client needs.
  • Collaborate with interdisciplinary teams to promote healthy lifestyle changes through shared decision-making practices.
  • Deliver medication management, including administration, monitoring, and coordination with pharmacies and medical providers.
  • Provide advocacy and follow-up care for hospitalized clients, ensuring continuity of care.
  • Educate clients and staff on good health practices and harm reduction strategies.
  • Respond to medical emergencies, documenting and addressing health concerns promptly.
  • Maintain accurate records, logs, and documentation to ensure compliance with policies and regulations.
  • Act as a liaison between external healthcare providers and community organizations to address client medical needs.

Registered Nurse Key Responsibilities & Duties

  • Carry a caseload and conduct weekly visits for clients in shelters, residential settings, and community locations.
  • Develop treatment plans addressing medical diagnoses and educate clients on health conditions.
  • Administer prescribed treatments and medications, monitoring for adverse reactions.
  • Coordinate medication delivery, refills, and insurance authorizations, ensuring timely administration.
  • Provide health education and training to clients and staff to support continuous health improvement.
  • Respond to medical emergencies, performing necessary interventions and documentation.
  • Monitor lab results and communicate changes to medical professionals for appropriate adjustments.
  • Participate in interdisciplinary mental health team meetings as the medical representative.
  • Maintain logs for injections, bloodwork, and other health monitoring tasks.

Registered Nurse Job Requirements

  • Bachelor of Science in Nursing (BSN) and New York State RN registration required.
  • Minimum of 1 year of nursing experience, preferably in behavioral health or residential settings.
  • Proficiency in electronic health records and Microsoft Office applications.
  • Strong knowledge of harm reduction frameworks and shared decision-making practices.
  • Ability to conduct health assessments, administer medications, and manage medical case files.
  • Excellent communication and advocacy skills to support client health needs.
  • Experience coordinating care with external healthcare providers and community organizations.
  • Capability to respond effectively to medical emergencies and document interventions.
