Job Description

Registered Nurse Overview

  • The Registered Nurse will provide comprehensive care and treatment to clients within a behavioral health-focused program.
  • Collaborate with interdisciplinary teams to develop and implement individualized treatment plans.
  • Conduct initial and ongoing health assessments, including vitals and physical health evaluations.
  • Deliver medical case management and medication administration in accordance with prescribed orders.
  • Promote healthy lifestyle changes through education and shared decision-making practices.
  • Provide health education and training to clients and staff to ensure continuous support.
  • Act as a liaison between clients, healthcare providers, and community organizations to coordinate care.
  • Respond to medical emergencies and ensure compliance with internal and external regulations.

Registered Nurse Key Responsibilities & Duties

  • Carry a caseload and perform regular nursing assessments for assigned clients.
  • Administer prescribed medications, including long-acting injections, and monitor for side effects.
  • Coordinate and deliver medications to client residences or shelters as needed.
  • Make referrals, schedule appointments, and accompany clients to medical visits when required.
  • Provide advocacy and follow-up care for hospitalized clients to ensure recovery.
  • Document all client interactions and maintain accurate records within electronic health systems.
  • Monitor lab results and communicate findings to medical providers for necessary adjustments.
  • Participate in interdisciplinary team meetings as the medical representative.
  • Train staff on health practices and emergency response procedures.

Registered Nurse Job Requirements

  • Bachelor of Science in Nursing (BSN) with New York State registration.
  • Minimum of 1 year of experience; 2 years preferred, especially in behavioral health.
  • Proficiency in electronic health records and Microsoft Office Suite (Word, Excel, Outlook).
  • Strong understanding of medical case management and medication administration.
  • Ability to work on-site and conduct community-based visits as required.
  • Experience in a residential or mental health setting is highly desirable.
  • Excellent communication skills to collaborate with clients, staff, and external providers.
  • Commitment to promoting health and wellness in a harm reduction framework.
