Team Manager – Hospice Overview

  • The Team Manager – Hospice will oversee nursing staff, ensuring high-quality hospice and palliative care services aligned with program goals and policies.
  • This hybrid role requires 3-4 days in-office, with flexibility for weekend coverage and weekday adjustments.
  • Candidates must possess supervisory experience and expertise in hospice, hospital AOC, or after-hours/on-call scenarios.
  • Availability beyond core hours is expected to address incidents and ensure patient care continuity.
  • The role involves mentoring RN Case Managers in pain management and patient-centered care strategies.
  • Community engagement through presentations alongside Admissions/Outreach Departments is part of the responsibilities.
  • This position offers the opportunity to lead impactful initiatives within a collaborative interdisciplinary team environment.
  • A retention bonus of $15,000 paid over three years is included.

Team Manager – Hospice Key Responsibilities & Duties

  • Provide leadership and guidance to nursing staff, ensuring adherence to hospice care philosophy and program policies.
  • Coordinate staff assignments in collaboration with the Associate Director of Patient Services to meet staffing ratios.
  • Mentor RN Case Managers in pain and symptom management, focusing on patient and family goals of care.
  • Monitor and evaluate nursing documentation for compliance with policies and regulations.
  • Participate in community presentations to promote hospice services and outreach efforts.
  • Ensure availability to address incidents beyond core hours, maintaining patient care standards.
  • Collaborate effectively within an interdisciplinary team to achieve program objectives.
  • Support the Associate Director in operational and strategic initiatives for hospice services.

Team Manager – Hospice Job Requirements

  • Graduate from an accredited School of Nursing; BSN or BS degree preferred.
  • Minimum of five years of hospice nursing experience with supervisory or leadership experience preferred.
  • New York State RN licensure required; CHPN certification preferred or obtainable within one year.
  • Proficiency in Word, Outlook, and Windows applications is essential.
  • Strong ability to supervise and mentor hospice nursing staff effectively.
  • Experience in triage, on-call, or hospital AOC scenarios is highly valued.
  • Capability to work within an interdisciplinary team and adapt to flexible scheduling needs.
  • Excellent communication and leadership skills to drive team performance and patient care excellence.
