Licensed Practical Nurse Key Responsibilities & Duties

• Conduct health-focused sessions with residents to identify and achieve wellness goals.
• Facilitate health and wellness groups to promote education and engagement.
• Perform health assessments, vital sign monitoring, and evaluations.
• Coordinate care with physicians, pharmacies, and treatment providers.
• Document client care and maintain medication administration records.
• Participate in treatment team meetings and case conferences.
• Provide first aid and CPR assistance during medical emergencies.
• Supervise assigned staff and support program operations.
• Advocate for residents' health needs and preferences effectively.

Licensed Practical Nurse Job Requirements

• Active New York State Licensed Practical Nurse (LPN) license is required.
• Associate of Science (AS) degree in nursing or related field.
• Minimum of 1 year of experience; 3 years preferred.
• Experience with behavioral health, substance use, or community-based populations is advantageous.
• Strong communication and care coordination skills.
• Proficiency in documentation and medication management.
