Job Description

Overview

  • Provide compassionate nursing care in a dynamic adult day care environment, focusing on chronic conditions and vision impairments.
  • Collaborate with a multidisciplinary team to deliver comprehensive health services and education tailored to registrants' needs.
  • Conduct thorough assessments and develop personalized care plans to enhance registrants' health outcomes.
  • Coordinate healthcare services and maintain effective communication with registrants' private physicians and caregivers.
  • Ensure adherence to safety protocols and infection control standards within the facility.
  • Provide health education and counseling to registrants and their families, promoting informed health decisions.
  • Supervise nursing students and volunteers, fostering professional growth and development.
  • Support registrants with daily activities, including escorting and meal service, ensuring their comfort and dignity.

Key Responsibilities & Duties

  • Perform comprehensive nursing assessments and document findings accurately to inform care plans.
  • Develop, implement, and evaluate individualized nursing care plans to meet registrants' health needs.
  • Monitor medication administration and ensure compliance with inventory and safety standards.
  • Respond promptly to medical emergencies and educate staff on emergency protocols.
  • Maintain a clean and safe environment, ensuring medical equipment is in optimal condition.
  • Lead health education sessions and provide one-on-one counseling to registrants.
  • Participate in quality assurance and performance improvement initiatives to enhance care standards.
  • Assist registrants with transportation and personal care needs, ensuring dignity and respect.
  • Act as Program Director in their absence, maintaining seamless program operations.

Job Requirements

  • Bachelor of Science in Nursing (BSN) from an accredited institution.
  • Current Registered Nurse license with 3-5 years of relevant experience.
  • Proficiency in MS Word and preferred certification in UAS-NY.
  • Experience in Med/Surg, Long Term Care, or Community Health settings.
  • Strong communication, documentation, and assessment skills.
  • Physical ability to lift and transfer registrants safely and effectively.
  • Commitment to confidentiality and adherence to healthcare information security policies.
  • Empathy and enthusiasm for working with adult and elderly populations.
  • Flexibility to perform interdisciplinary team tasks and general assistance duties.
