Job Description

Nurse Practitioner Overview

  • As a Nurse Practitioner, provide comprehensive healthcare services to residents in a dynamic and collaborative environment.
  • Conduct health assessments, physical examinations, and medication reconciliation to ensure optimal patient care.
  • Develop individualized care plans in collaboration with interdisciplinary teams, addressing unique needs and preferences.
  • Manage chronic conditions and acute illnesses, providing timely interventions and referrals as necessary.
  • Order and interpret diagnostic tests to support accurate diagnosis and treatment planning.
  • Prescribe medications and treatments within scope of practice and regulatory guidelines.
  • Provide education and counseling to residents and families on health maintenance and disease prevention.
  • Participate in quality improvement initiatives to enhance care quality and resident outcomes.
  • Maintain accurate documentation of assessments, interventions, and communications in electronic medical records.

Nurse Practitioner Job Requirements

  • Master’s degree in Nursing and completion of an accredited Nurse Practitioner program.
  • Unrestricted New York State Nurse Practitioner license.
  • Valid DEA permit for prescribing medications within scope of practice.
  • Eligibility to provide reimbursable services under Medicare and Medicaid.
  • Minimum of 2 years of clinical experience, with 3 years preferred.
  • Experience in long-term care settings is highly desirable.
  • Proficiency with electronic health records, preferably eClinicalWorks (eCW).
  • Strong interpersonal and communication skills for effective patient and team interactions.
