Community Liaison Key Responsibilities & Duties

• Establish and maintain relationships with physicians, healthcare providers, and community organizations to promote hospice services.
• Conduct outreach and educational initiatives to increase awareness and preference for hospice care.
• Collaborate with referral sources to facilitate patient access to compassionate end-of-life care.
• Lead initial conversations with patients and families to explain hospice services and philosophy.
• Drive census growth by identifying new referral opportunities and strengthening existing partnerships.
• Ensure compliance with organizational standards and maintain accurate records of outreach activities.
• Represent the client at community events to promote hospice care and build brand awareness.
• Provide feedback to internal teams to improve service delivery and outreach effectiveness.

Community Liaison Job Requirements

• Bachelor’s degree in a related field preferred; equivalent experience may be considered.
• Minimum of two years of healthcare sales or marketing experience required; hospice marketing experience preferred.
• Proven ability to build and maintain professional relationships with healthcare providers and community organizations.
• Strong communication and interpersonal skills to effectively advocate for hospice services.
• Reliable transportation, valid driver’s license, and auto insurance required for fieldwork.
• Ability to work in a hybrid environment, balancing field visits with administrative responsibilities.
• Commitment to the mission of providing compassionate, patient-centered end-of-life care.
• Proficiency in using CRM tools and maintaining accurate outreach records.
