  • The Director of Case Management oversees patient care coordination and case management within an ambulatory healthcare setting.
  • Leads strategic initiatives to enhance patient care, compliance, and operational efficiency.

Director of Case Management Key Responsibilities & Duties

  • Lead and manage the case management department, ensuring efficient operations and compliance.
  • Develop and implement strategic plans for patient care coordination and resource management.

  • Current NYS Licensure as a Registered Professional Nurse or Social Worker required.
  • Certification in Case Management and PRI Certification preferred.
  • Strong understanding of patient care assessment, planning, and management.
  • Familiarity with state and regulatory requirements related to case management.
  • Proven ability to lead teams and drive performance improvements.
