Registered Nurse Admissions Overview

• As a Registered Nurse Admissions, you will oversee patient referrals and admissions into the hospice program, ensuring timely and safe transitions.
• Collaborate with interdisciplinary teams, including physicians, social workers, and nurses, to coordinate care plans effectively.
• Monitor and manage high-tech and complex referrals in the Access Center for hospice admissions.
• Verify all written and verbal orders for evaluation prior to admitting patients into the program.
• Provide excellent customer service and communication to patients and their families during the admission process.
• Work on-site with flexible hours, including occasional weekend shifts, to meet patient needs.
• Utilize your bilingual skills to assist diverse patient populations effectively, with preference for Chinese language proficiency.
• Access to a reliable vehicle is required for travel between locations within Brooklyn and Queens.

Registered Nurse Admissions Key Responsibilities & Duties

• Manage referrals and admissions into the hospice program, ensuring compliance with all protocols and standards.
• Collaborate with Access Coordinators to maintain an updated referral list and ensure timely admissions.
• Communicate effectively with interdisciplinary teams to coordinate patient care and ensure safe transitions.
• Verify written and verbal orders for evaluation prior to patient admission into the hospice program.
• Provide exceptional customer service to patients and families, addressing concerns and ensuring satisfaction.
• Utilize computer skills to document patient information and manage admissions records accurately.
• Participate in team meetings and contribute to continuous improvement initiatives within the Access Center.
• Adhere to all regulatory and organizational policies and procedures during patient admissions.

Registered Nurse Admissions Job Requirements

• Graduate from an accredited school of Nursing with an Associate of Applied Science (AAS) degree; BSN preferred.
• Current New York State RN license in good standing.
• Minimum of 2 years of acute care experience; hospice experience preferred.
• Proficiency in PC skills and ability to document patient information accurately.
• Strong written and oral communication skills to interact effectively with patients and teams.
• Bilingual proficiency in Chinese is preferred to assist diverse patient populations.
• Access to a reliable vehicle for travel within Brooklyn and Queens.
• Availability to work flexible hours, including one weekend shift per month.
