Case Manager Overview
- The Case Manager ensures quality care by promoting cost-effective outcomes for patients.
- Provide case management services to children, families, and adults tailored to their specific needs.
- Collaborate with multidisciplinary teams to ensure comprehensive care and positive health outcomes.
- Engage patients and families to build trusting relationships and resolve crises effectively.
- Manage over 100 cases in a fast-paced environment, ensuring timely and detailed communication.
- Conduct assessments and develop individualized care plans for patients, ensuring access to necessary services.
- Promote health education and self-management strategies during primary care visits.
- Represent the organization at events to recruit patients and promote services.
Case Manager Key Responsibilities & Duties
- Provide proactive primary case management, including intake, assessment, and care plan development.
- Coordinate with multidisciplinary teams to ensure continuity of care for patients.
- Link patients to additional services such as specialty care, entitlements, and mental health support.
- Respond promptly to inquiries and referrals, ensuring timely communication and follow-up.
- Maintain accurate and updated patient records, meeting all documentation requirements.
- Participate in internal and external meetings and training sessions as required.
- Assist patients in obtaining prior authorizations for pharmacy and medical services.
- Adhere to organizational policies and compliance requirements at all levels.
Case Manager Job Requirements
- Bachelor’s degree in a related field with 2 years of qualifying experience required.
- Experience working with target populations such as children, families, and individuals with specific health conditions preferred.
- UAS-NY certification required within one month of hire.
- Fluency in Spanish or an Asian language preferred.
- Proficiency in various applications such as Outlook, Word, Excel, and healthcare-specific software required.
- Strong communication skills and ability to work in collaborative and individual settings.
- Ability to work flexible hours, including evenings and weekends.
- Excellent customer service skills to address patient and provider concerns effectively.
