Bilingual Case Manager Overview

  • The Bilingual Case Manager serves as the primary liaison between members and the plan, ensuring effective communication and service coordination.
  • This role involves reviewing person-centered service plans and coordinating covered and non-covered benefits for members.
  • Engage in monthly communication and follow-ups with members to address identified needs and concerns.
  • Document interactions and updates thoroughly in member charts, maintaining accuracy and timeliness.
  • Participate in weekly team meetings to discuss member care and operational strategies.
  • Work in a hybrid schedule, combining on-site and remote work for optimal flexibility.
  • Utilize basic Excel skills for document management, filtering, sorting, and saving files.
  • Leverage bilingual skills in Chinese, Spanish, Bengali, Urdu, or Gujarati to effectively communicate with diverse members.

Bilingual Case Manager Key Responsibilities & Duties

  • Conduct monthly communications with members to ensure their needs are addressed and service plans are reviewed.
  • Coordinate member care with family members, vendor agencies, and internal teams for seamless service delivery.
  • Document member interactions and updates in charts, ensuring compliance with organizational standards.
  • Participate in weekly team meetings to discuss strategies and address member care concerns.
  • Perform basic Excel tasks, including document hiding, saving, filtering, and sorting.
  • Provide bilingual support in Chinese, Spanish, Bengali, Urdu, or Gujarati to enhance communication effectiveness.
  • Ensure timely follow-up and response to identified items requiring attention for member care.
  • Maintain knowledge of member assessment and clinical review procedures to support effective case management.

Bilingual Case Manager Job Requirements

  • Associate of Arts (AA) degree required; MSW or RN preferred for advanced case management knowledge.
  • Minimum of 1 year of case management experience, with bilingual proficiency in Chinese, Spanish, Bengali, Urdu, or Gujarati.
  • Proficiency in basic Excel functions, including document management, filtering, and sorting.
  • Strong ability to communicate effectively with members of diverse ages and socioeconomic backgrounds.
  • Knowledge of member assessment procedures and clinical review processes.
  • Demonstrated ability to read, interpret, and effectively communicate information to members and agency employees.
  • Hybrid work schedule requiring on-site presence three days a week and remote work two days.
  • Commitment to thorough and timely documentation in member charts and adherence to organizational standards.
