Patient Care Coordinator RN Key Responsibilities & Duties
- Conduct pre-visit planning and review daily patient appointments for optimal care coordination.
- Prepare and analyze patient rosters to identify care priorities.
- Coordinate and document patient care conferences to ensure comprehensive care delivery.
- Develop individualized care plans in collaboration with healthcare providers.
- Educate patients on care plans and assist in overcoming barriers to treatment.
- Document patient interactions and care plans using eClinicalWorks (eCW).
- Collaborate with Benefits Counselors, Outreach staff, and HIM for scheduling and documentation.
- Participate in Quality Improvement meetings to enhance care processes.
Patient Care Coordinator RN Job Requirements
- Bachelor of Science in Nursing (BSN) and valid New York State RN license required.
- Minimum of 1 year clinical experience; 3 years preferred.
- Proficiency in using EMR systems for patient documentation and scheduling.
- Strong administrative skills to manage care conferences and patient outreach.
- Comfortable with Quality Improvement processes and team collaboration.
- Excellent communication skills to educate patients and coordinate care plans.
- Ability to work on-site Monday through Friday, 9:00 AM to 5:00 PM.
