• Work full-time, Monday to Friday, with on-call duties one weekend per month.

Home Care Registered Nurse Key Responsibilities & Duties

• Provide direct nursing care to patients, addressing acute needs and promoting recovery.
• Case manage assigned patients to achieve positive clinical outcomes and minimize rehospitalization risks.
• Collaborate with physicians to ensure comprehensive and effective care delivery.
• Perform fieldwork in the Bronx region, requiring reliable transportation and a valid driver’s license.
• Participate in on-call duties one weekend per month, ensuring continuity of care.

Home Care Registered Nurse Job Requirements

• Associate of Applied Science (AAS) in Nursing; BSN strongly preferred.
• Active Registered Nurse (RN) license required; must pass a pharmacy test.
• Minimum of 2 years of nursing experience; hospital, acute, or home care experience preferred.
• Proficiency in case management and interdisciplinary care planning.
• Strong communication skills to collaborate with caregivers, families, and healthcare professionals.
• Reliable transportation and a valid driver’s license to perform fieldwork in the Bronx region.
• Commitment to providing compassionate, patient-centered care in a field-based environment.
• Ability to work full-time, Monday to Friday, with occasional weekend on-call duties.
