Registered Nurse Case Manager Overview

  • The Registered Nurse Case Manager oversees nursing care for individuals with developmental disabilities, ensuring compliance with medical standards and regulations.
  • Collaborates with interdisciplinary teams to develop and implement individualized care plans tailored to each consumer's needs.
  • Provides direct nursing care and supervises support staff in performing medical procedures and tasks.
  • Offers training to staff on health measures, medication administration, and specialized care techniques.
  • Ensures accurate and timely documentation of medical records and nursing notes.
  • Monitors health and behavioral status, coordinating acute and ongoing health services as needed.

  • Utilizes electronic health record systems for care coordination and communication with external providers.

Registered Nurse Case Manager Key Responsibilities & Duties

  • Conduct nursing assessments and develop individualized care plans for consumers, updating them annually or as needed.
  • Supervise and train staff on health and hygiene practices, communicable disease control, and medical procedures.
  • Coordinate health services and ensure compliance with state and federal regulations.
  • Monitor and document health status, lab results, and treatment efficacy, communicating findings to clinical personnel.
  • Provide training and support for medication administration, tube feeding, and diabetic care.
  • Supervise Licensed Practical Nurses (LPNs) and direct support staff, conducting performance evaluations and scheduling.
  • Participate actively in interdisciplinary team meetings to ensure comprehensive care coordination.
  • Maintain documentation of training programs, attendance, and post-training assessments.
  • Ensure readiness for on-call responsibilities, providing backup support as needed.

Registered Nurse Case Manager Job Requirements

  • Associate of Applied Science (AAS) degree in Nursing or equivalent educational background.
  • Minimum of 1 year of experience in nursing, preferably with OPWDD or MRDD populations.
  • Proficiency in electronic health record management and care coordination systems.
  • Strong knowledge of OPWDD rules, regulations, and nursing standards.
  • Ability to supervise and train staff in medical procedures and preventive health measures.
  • Excellent documentation and communication skills for accurate medical recordkeeping.
  • Flexibility to adapt to varying schedules and on-call responsibilities.
