Social Worker Overview

• The Social Worker provides clinical behavioral health services to patients of all ages, ensuring high-quality care and support.
• Conduct psychological assessments, diagnostic evaluations, and therapeutic interventions tailored to individual patient needs.
• Collaborate with multidisciplinary teams to develop and implement comprehensive treatment plans.
• Engage patients in their treatment planning, promoting education about their conditions and fostering empowerment.
• Provide crisis intervention, including emergency support and risk management strategies.
• Maintain accurate and timely documentation in electronic medical records (EMR).
• Contribute to quality improvement initiatives and policy development within the behavioral health department.
• Work within the scope of licensure and adhere to established standards of care and regulatory guidelines.
• Participate in professional development opportunities to enhance skills and knowledge.

Social Worker Key Responsibilities & Duties

• Conduct psychosocial assessments and assign diagnoses using DSM-5 and ICD-10 criteria.
• Develop and implement individualized treatment plans in collaboration with patients and healthcare teams.
• Provide psychotherapy and psychoeducation using individual, family, and group modalities.
• Facilitate referrals and coordinate services while respecting patient confidentiality.
• Deliver crisis intervention services, including emergency room escort and suicide risk management.
• Collaborate with primary care providers to ensure integrated care delivery.
• Participate in multidisciplinary team meetings and case conferences to discuss patient care.
• Maintain accurate documentation and complete required reports promptly.
• Contribute to the development and review of behavioral health policies and procedures.

Social Worker Job Requirements

• Master of Social Work degree and valid New York State licensure (LCSW, LMHC, LMFT).
• Minimum of 2 years of experience in mental health services, preferably with diverse populations.
• Proficiency in electronic medical records (eClinicalWorks preferred) and Microsoft Office.
• Strong organizational skills for managing assessments and treatment plans effectively.
• Excellent written and verbal communication skills, with the ability to build rapport quickly.
• Experience working with multicultural populations and Limited English Proficient individuals.
• Fluency in Spanish or an Asian language is a plus.
• Ability to work flexible hours, including evenings and weekends as needed.
