• The Licensed Clinical Social Worker (LCSW) will provide comprehensive psychosocial assessments and therapeutic services to individuals and couples in a clinical setting.
• Collaborate closely with psychiatrists and other care providers to ensure cohesive treatment plans and patient care.
• Prepare and maintain detailed documentation including assessments, care plans, updates, and discharge notes in compliance with organizational policies.
• Participate in bi-monthly group supervision sessions to enhance professional development and clinical practice.
• Educate patients about the Patient-Centered Medical Home (PCMH) program, promoting its values and goals effectively.
• Attend required in-service education programs to stay updated on clinical practices and standards.
• Maintain active licensing status in New York State by completing continuing education requirements as mandated.
• Travel between clinic sites as needed to meet scheduling and patient care demands.

Licensed Clinical Social Worker Key Responsibilities & Duties

• Conduct psychosocial assessments and provide individual and couple psychotherapy sessions tailored to patient needs.
• Develop and update treatment care plans, ensuring timely documentation of patient progress and outcomes.
• Collaborate with multidisciplinary teams to deliver integrated care and optimize patient treatment plans.
• Provide patients with relevant information and resources to support their understanding of clinical programs and services.
• Participate in supervision and training sessions to enhance clinical skills and knowledge.
• Ensure compliance with organizational policies and procedures in all aspects of clinical practice.
• Maintain proficiency in using electronic medical records and practice management software.
• Provide crisis intervention and support as needed to address urgent patient needs.

Licensed Clinical Social Worker Job Requirements

• Master’s Degree in Social Work from an accredited institution in the United States.
• Minimum of 2 years of clinical experience in a therapeutic setting.
• Active New York State Licensed Clinical Social Worker (LCSW) certification.
• Proficiency in Microsoft Office Suite and electronic medical record systems.
• Strong communication skills to effectively interact with patients and multidisciplinary teams.
• Ability to travel between clinic sites based on scheduling needs.
• Preferred certifications include SCIP-R and CPR training.
• Commitment to completing continuing education requirements to maintain licensing status.
