Job Description

Registered Nurse Overview

  • The Registered Nurse (RN) will provide multidisciplinary health services for older adults in a dynamic Adult Day Care Center.
  • Coordinate care plans with physicians and healthcare teams to ensure optimal patient outcomes and adherence to care protocols.
  • Conduct comprehensive clinical assessments to identify and address registrants' service needs effectively.
  • Administer medications, prefill prescriptions, and perform skilled nursing services as per physician orders.
  • Monitor registrants' health status and adjust care plans in collaboration with multidisciplinary teams.
  • Educate registrants and their families on disease processes, self-care techniques, and preventive strategies.
  • Document all services provided, ensuring compliance with clinical and regulatory standards.
  • Participate in team meetings to report progress and adherence to care plans.
  • Contribute to a supportive and engaging environment for registrants and staff.

Registered Nurse Key Responsibilities & Duties

  • Conduct clinical assessments and develop nursing care plans for registrants upon intake and quarterly.
  • Ensure timely completion and documentation of MD orders and nursing evaluations.
  • Administer medications and perform skilled nursing services as outlined in care plans.
  • Assign and oversee CAN tasks to ensure daily completion and adherence to care plans.
  • Collaborate with physicians and clinical teams to provide cost-efficient patient care.
  • Monitor registrants' health status and communicate changes to physicians and teams.
  • Educate registrants and families on health management and preventive care strategies.
  • Participate in multidisciplinary meetings to report progress and adherence to care plans.
  • Maintain accurate documentation of all services provided and care plan adjustments.

Registered Nurse Job Requirements

  • Current license and registration as a Registered Professional Nurse in New York State.
  • Bachelor of Science in Nursing (BSN) from an accredited institution preferred.
  • Minimum one year of experience in acute or subacute care settings required.
  • Experience in Adult Care Centers and geriatric care preferred.
  • Bilingual proficiency in Spanish and English is advantageous.
  • UAS certification is preferred for enhanced care delivery.
  • Strong clinical assessment and diagnosis coding skills.
  • Effective communication and collaboration with multidisciplinary teams.
