Registered Nurse Overview
- The Registered Nurse (RN) provides comprehensive nursing services across crisis respite and transitional programs.
- Support individuals with dual diagnoses of intellectual disabilities and severe mental illness through skilled nursing care.
- Collaborate with interdisciplinary teams to promote health, safety, and successful community integration.
- Ensure complex medical, behavioral, and psychiatric needs are met during inpatient-to-community transitions.
- Provide direct nursing interventions, including chronic disease management and crisis stabilization.
- Monitor medication administration, therapeutic responses, and document health outcomes effectively.
- Deliver health education to individuals, families, and staff to promote independence and wellness.
- Maintain compliance with OSHA mandates and ensure infection control guidelines are followed.
Registered Nurse Key Responsibilities & Duties
- Conduct initial and ongoing nursing assessments for individuals admitted to programs.
- Develop and monitor health goals tailored to individual needs and update as required.
- Screen individuals for program admission and participate in intake team activities.
- Facilitate hospital discharge and community reintegration ensuring continuity of care.
- Coordinate follow-up care with primary care providers, specialists, and community resources.
- Train and mentor direct support professionals on health-related tasks and emergency response.
- Serve as the clinical liaison among providers, families, and program staff.
- Maintain timely and accurate documentation in compliance with regulations.
- Ensure appropriate medication storage and disposal procedures are followed.
Registered Nurse Job Requirements
- Bachelor of Science in Nursing (BSN) and current New York State RN license.
- Minimum two years of nursing experience; background in IDD or behavioral health preferred.
- Familiarity with OPWDD, OMH, and DOH nursing regulations and program requirements.
- Strong clinical assessment, crisis intervention, and care coordination skills.
- Excellent communication, organizational, and documentation abilities.
- Willingness to travel throughout the Five Boroughs for program needs.
- Ability to work independently while thriving in a team-based environment.
- Valid driver’s license preferred for travel to multiple program sites.
