Physical Therapist Key Responsibilities & Duties

• Conduct in-home evaluations and develop customized treatment plans for patients.
• Administer therapy sessions focusing on mobility, balance, strength, and pain management.
• Collaborate with referring physicians, healthcare teams, and caregivers to optimize patient outcomes.
• Maintain accurate and timely documentation in various EMR systems.
• Educate patients and caregivers on effective home management techniques.
• Schedule therapy appointments and coordinate with patients for convenient timings.

Physical Therapist Job Requirements

• Active Florida Physical Therapist License is required.
• Minimum of 1 year of experience in home health therapy is preferred.
• Proficiency in multiple EMR systems, including Pariox, Kantime, and Kinnser.
• High School Diploma or GED is required for this position.
• Valid driver’s license and reliable transportation are essential.
• Strong organizational and documentation skills to manage patient schedules effectively.
