Occupational Therapist Overview

  • The Occupational Therapist will provide personalized rehabilitation services to patients in their homes.

Occupational Therapist Key Responsibilities & Duties

  • Conduct clinical assessments to determine patient needs and therapy appropriateness.
  • Develop, document, and revise individualized care plans based on ongoing evaluations.

Occupational Therapist Job Requirements

  • Proficiency in multiple EMR systems such as Kantime, Kinnser, Axxess, and Homecare Homebase.
  • High School Diploma or GED is required for educational qualification.
  • Reliable transportation and a valid driver’s license are essential.
  • Strong organizational and documentation skills to manage patient records effectively.
  • Professional demeanor and commitment to delivering exceptional patient care.
