Job Description

Registered Nurse Overview

  • The Registered Nurse will provide specialized diabetes care and education to members, focusing on improving health outcomes and reducing complications.
  • Collaborate with the Quality Assurance RN Manager to address gaps in diabetes care and support members with high A1Cs.
  • Engage members telephonically to develop personalized management plans that align with their lifestyle and cultural needs.
  • Educate members on diabetes self-management, including nutrition, physical activity, medication adherence, and glucose monitoring.
  • Provide guidance on using diabetes-related devices, such as insulin pens and glucose monitors.
  • Support members in coping with lifestyle changes and reducing emergency room visits through proactive care strategies.
  • Contribute to quality assurance and improvement processes within the Wellness-MAP Department.
  • Perform additional duties and projects as assigned by management to enhance member care.

Registered Nurse Key Responsibilities & Duties

  • Develop and implement diabetes management plans tailored to individual member needs and physician collaboration.
  • Provide comprehensive diabetes education, addressing lifestyle, cultural, and coping strategies.
  • Assist members in identifying patterns and opportunities for improvement in daily self-management.
  • Educate members on the use of prescribed diabetes devices and monitoring tools.
  • Conduct telephonic outreach to members and providers to address gaps in diabetes care.
  • Collaborate with the Quality Assurance RN Manager to ensure high standards of care delivery.
  • Maintain accurate documentation and reporting for quality assurance purposes.
  • Participate in ongoing training and professional development to enhance care delivery.

Registered Nurse Job Requirements

  • Valid New York State Registered Nurse (RN) license is required.
  • Bachelor’s degree in Nursing (BSN) is preferred.
  • Certification as Diabetes Care and Education Specialist/Certified Diabetes Educator is mandatory.
  • Minimum of three years of experience in diabetes care and education.
  • Two years of nursing experience in ambulatory care, community health, or medical/surgical settings.
  • Strong skills in clinical assessment, patient education, and motivational interviewing.
  • Intermediate proficiency in Microsoft Outlook, Word, and Excel is preferred.
  • Bilingual in Spanish and a valid NYS Driver’s license are preferred.
