Nurse Director Case Management Overview

  • The Nurse Director Case Management oversees hospital-wide case management operations, ensuring compliance with policies and external regulations.
  • Leads interdisciplinary teams to optimize patient care and discharge planning processes, ensuring continuity of care and efficient resource utilization.
  • Manages departmental activities, including staffing, onboarding, and performance evaluations, to achieve operational and quality objectives.
  • Collaborates with hospital leadership and external stakeholders to align case management initiatives with organizational goals.
  • Serves as a subject matter expert in case management, providing strategic direction and leadership to the department.

  • Minimum 3 years of experience in acute care hospital case management required; 5 years preferred.
