Registered Nurse Overview
- The Registered Nurse will provide skilled hospice and home-based nursing care to patients in their homes.
- This field-based role focuses on individualized patient care within Bergen, Passaic, and Hudson Counties.
- Collaborate with physicians and interdisciplinary teams to ensure safe, compassionate care delivery.
- Perform comprehensive patient assessments, develop care plans, and implement treatments.
- Provide patient and caregiver education tailored to individual care goals.
- Maintain accurate clinical documentation and ensure compliance with physician orders.
- Travel throughout assigned territories to deliver care in patients' homes.
- Opportunity to work independently and manage a patient caseload effectively.
Registered Nurse Key Responsibilities & Duties
- Provide professional nursing care to hospice and home care patients.
- Conduct patient assessments and create individualized care plans.
- Perform skilled nursing procedures and treatments as prescribed.
- Monitor patient conditions and communicate changes to physicians.
- Educate patients and caregivers on care goals and treatments.
- Maintain compliance with insurance authorizations and payor requirements.
- Coordinate care with interdisciplinary team members and make referrals as needed.
- Ensure accurate and timely clinical documentation of patient care.
Registered Nurse Job Requirements
- Active NJ Registered Nurse License and AHA BLS/HCP Certification required.
- Associate's Degree in Nursing or Nursing Diploma; BSN preferred.
- Minimum 6+ months RN experience or 1+ year LPN experience.
- Home care or hospice experience strongly preferred.
- Strong clinical judgment, critical-thinking, and documentation skills required.
- Ability to work independently in a field-based environment.
- Must be comfortable traveling to patients' homes within assigned territories.
- Must be able to lift, push, and pull up to 50 lbs.
