Registered Nurse Overview

• The Registered Nurse will provide skilled hospice and home-based nursing care to patients in their homes.
• This field-based role focuses on individualized patient care within Bergen, Passaic, and Hudson Counties.
• Collaborate with physicians and interdisciplinary teams to ensure safe, compassionate care delivery.
• Perform comprehensive patient assessments, develop care plans, and implement treatments.
• Provide patient and caregiver education tailored to individual care goals.
• Maintain accurate clinical documentation and ensure compliance with physician orders.
• Travel throughout assigned territories to deliver care in patients' homes.
• Opportunity to work independently and manage a patient caseload effectively.

Registered Nurse Key Responsibilities & Duties

• Provide professional nursing care to hospice and home care patients.
• Conduct patient assessments and create individualized care plans.
• Perform skilled nursing procedures and treatments as prescribed.
• Monitor patient conditions and communicate changes to physicians.
• Educate patients and caregivers on care goals and treatments.
• Maintain compliance with insurance authorizations and payor requirements.
• Coordinate care with interdisciplinary team members and make referrals as needed.
• Ensure accurate and timely clinical documentation of patient care.

Registered Nurse Job Requirements

• Active NJ Registered Nurse License and AHA BLS/HCP Certification required.
• Associate's Degree in Nursing or Nursing Diploma; BSN preferred.
• Minimum 6+ months RN experience or 1+ year LPN experience.
• Home care or hospice experience strongly preferred.
• Strong clinical judgment, critical-thinking, and documentation skills required.
• Ability to work independently in a field-based environment.
• Must be comfortable traveling to patients' homes within assigned territories.
• Must be able to lift, push, and pull up to 50 lbs.
