Palliative Care Nurse Practitioner or Physician Assistant Key Responsibilities & Duties
- Conduct inpatient palliative care consultations and follow-up visits across various hospital units.
- Perform holistic, evidence-based assessments to manage complex symptoms for patients with serious illnesses.
- Lead discussions on goals-of-care and advance-care planning with patients and families.
- Coordinate with post-acute resources, including SNFs and hospice providers, as needed.
- Collaborate closely with physicians, APPs, social workers, and care coordination teams.
- Ensure timely documentation aligned with clinical, billing, and quality standards.
- Play an active role in discharge and transition planning based on patient needs.
- Participate in quality improvement initiatives and contribute to the palliative medicine practice.
Palliative Care Nurse Practitioner or Physician Assistant Job Requirements
- Master of Science in Nursing or equivalent degree required.
- Active New Jersey Nurse Practitioner or Physician Assistant license mandatory.
- National board certification and DEA/CDS registration required.
- Minimum of 2 years of palliative care experience; 5 years preferred.
- Experience in inpatient or hospital-based palliative care strongly preferred.
- Palliative care certification (ACHPN/CHPN) is highly desirable.
- Proficiency in managing complex symptoms and working with high-acuity patients.
- Strong clinical judgment, communication skills, and a compassionate approach to patient care.
