Licensed Practical Nurse Overview

• The Licensed Practical Nurse (LPN) will provide medical case management for women in supportive housing with mental health challenges.
• Collaborate with interdisciplinary teams to advocate for client wellness and independence.
• Focus on improving long-term health outcomes and stability through healthcare and psychiatric support.
• Conduct health assessments, wellness checks, and monitor vital signs for assigned clients.
• Coordinate care with physicians, pharmacies, and psychiatric providers to ensure continuity of care.
• Assist clients in navigating community resources and daily living challenges.
• Maintain accurate documentation, medication records, and controlled substance logs.
• Provide health education and advocate for client-centered healthcare goals.
• Respond to emergencies and provide first aid/CPR assistance as needed.

Licensed Practical Nurse Key Responsibilities & Duties

• Serve as the primary medical case manager for assigned clients, ensuring comprehensive care coordination.
• Conduct regular health assessments and wellness checks to monitor client progress.
• Coordinate with healthcare providers, pharmacies, and psychiatric teams for effective treatment plans.
• Support clients with medical, psychiatric, and psychosocial needs to promote stability.
• Assist clients in accessing community resources and overcoming practical life challenges.
• Maintain accurate and detailed documentation of client interactions and medication records.
• Participate in treatment team meetings and case conferences to develop care strategies.
• Provide health education and advocate for personalized healthcare goals for clients.
• Supervise assigned staff and contribute to overall site operations and efficiency.

Licensed Practical Nurse Job Requirements

• Active Licensed Practical Nurse (LPN) license required to practice.
• Associate of Applied Science (AAS) degree in nursing or related field.
• Minimum of 1 year of experience in healthcare; 3 years preferred.
• Strong interest in behavioral health, supportive housing, and community-based care.
• Ability to work effectively with vulnerable populations experiencing medical and psychiatric challenges.
• Excellent communication, documentation, and organizational skills.
• Proficiency in maintaining accurate chart documentation and medication records.
• Availability to work the specified schedule, including weekends.
• Knowledge of first aid and CPR techniques for emergency situations.
