Case Manager – Medical Overview
- The Case Manager – Medical will provide direct case management to patients, ensuring access to and retention in medical care.
- Collaborate with multidisciplinary teams to develop individualized care plans and achieve positive health outcomes.
- Coordinate patient access to specialty care, entitlements, and mental health services.
- Ensure accurate reporting and documentation in both electronic and paper records.
- Facilitate early access to treatment for specific communities, including Asians and Pacific Islanders living with HIV/AIDS.
- Work collaboratively within the organizational structure to meet quality improvement goals.
- Engage in proactive case management activities, including intake and reassessment processes.
- Maintain compliance with health codes and organizational policies regarding immunizations and disease prevention.
Case Manager – Medical Key Responsibilities & Duties
- Conduct comprehensive assessments and develop individualized care plans for patients.

Case Manager – Medical Job Requirements
- Bachelor’s degree in social science, psychology, social work, or health-related field required.
- Minimum of three years of relevant work experience in case management preferred.
- Experience working with communities such as people living with HIV/AIDS, immigrants, and LGBT individuals required.
- Fluency in one or more Asian languages is preferred.
- Proficiency in office software and electronic medical records systems required.
- Ability to work flexible hours, including evenings and weekends, as needed.
