Licensed Practical Nurse Overview

• The Licensed Practical Nurse (LPN) plays a vital role in the assessment center team, providing medical care and support to clients entering the shelter system.
• Collaborate with a multidisciplinary team including medical doctors, psychologists, and social workers to assess and determine appropriate placements for clients.
• Work in a dynamic environment focused on serving medically vulnerable populations, utilizing medical, psychiatric, and substance use expertise.
• Provide direct health education and advocacy for clients, helping them establish health goals and preferences.

Licensed Practical Nurse Key Responsibilities & Duties

• Supervise assigned staff and coordinate medical and psychosocial referrals for clients.
• Act as primary medical case manager, conducting health checks and optimizing client outcomes.
• Maintain medication administration records, controlled substance logs, and ensure adequate medication supply.
• Participate in treatment team meetings and case conferences to share findings and coordinate care.
• Provide emergency first aid and CPR assistance when required.
• Educate clients on health topics and advocate for their health preferences and goals.
• Collaborate with nursing leadership and team members to ensure comprehensive client care.
• Perform related duties as assigned to support the assessment center's mission.

Licensed Practical Nurse Job Requirements

• Valid Licensed Practical Nurse (LPN) certification is required.
• Minimum of 1 year of nursing experience; 2 years preferred.
• Associate of Applied Science (AAS) degree in nursing or related field.
• Experience in medical, psychiatric, and substance use care is highly beneficial.
• Strong teamwork and communication skills to collaborate effectively with multidisciplinary teams.
• Ability to manage medical records and medication administration logs accurately.
• Commitment to providing compassionate care to medically vulnerable populations.
• Availability to work onsite during scheduled hours with flexibility for emergencies.
