- The Team Manager – Hospice oversees nursing staff to ensure high-quality hospice care delivery.
- Provides leadership and mentoring to nursing staff, fostering excellence in patient care.

- Supports adherence to program policies and state and federal regulations.
- Schedules staff assignments to meet organizational and regulatory standards.

Team Manager – Hospice Key Responsibilities & Duties
- Lead and mentor nursing staff to ensure consistent delivery of hospice care.
- Collaborate with the Associate Director to schedule staff assignments effectively.
- Empower RN Case Managers in pain and symptom management practices.
- Monitor and evaluate nursing documentation for compliance and accuracy.
- Participate in interdisciplinary team meetings to align care strategies.
- Make community presentations to promote hospice services.
- Ensure adherence to state and federal regulations in hospice care delivery.
- Support orientation and training programs for new staff.
Team Manager – Hospice Job Requirements
- Bachelor of Science in Nursing (BSN) required; BS degree preferred.
- Minimum of five years of hospice nursing experience required.
- Supervisory or leadership experience preferred.
- New York State RN licensure required.
- National Board Certification in Hospice and Palliative Care (CHPN) preferred.
- Proficiency in Word, Outlook, and Windows applications required.
- Ability to supervise and mentor hospice nursing staff effectively.
- Strong teamwork skills within interdisciplinary team environments.
