Registered Nurse Case Manager Hospice Overview

  • As a Registered Nurse Case Manager Hospice, provide compassionate end-of-life care to patients in their homes.
  • Conduct physical and psychosocial assessments to develop tailored nursing care plans for hospice patients.
  • Collaborate with an interdisciplinary team to ensure holistic care delivery.
  • Supervise and train home health aides, homemakers, and personal care workers in patient care practices.
  • Request and manage physician orders to adapt care plans based on patient needs.
  • Provide bilingual support for Chinese-speaking patients to enhance communication and care quality.
  • Ensure accurate documentation of clinical data and patient progress using electronic systems.
  • Deliver education and support to patients and families regarding hospice care processes.
  • Maintain compliance with healthcare regulations and standards in hospice care.

Registered Nurse Case Manager Hospice Key Responsibilities & Duties
