Primary Care Family Nurse Practitioner Key Responsibilities & Duties

• Conduct thorough patient assessments, including histories and physical examinations, for individuals of all ages.
• Diagnose and treat acute and chronic conditions, developing individualized care plans.
• Prescribe medications and provide guidance on proper usage and potential side effects.
• Educate patients on preventive care, nutrition, family planning, and overall health maintenance.
• Perform well-care examinations, including gynecological assessments as needed.
• Coordinate referrals to specialists and ensure follow-up care is completed.
• Collaborate with healthcare professionals to optimize patient outcomes and care delivery.
• Participate in immunization programs and assist with phlebotomy when required.

Primary Care Family Nurse Practitioner Job Requirements

• Master of Science in Nursing with active Family Nurse Practitioner certification.
• Unrestricted New York State NP license and DEA registration.
• Minimum of 2 years of experience in family medicine or primary care.
• Experience treating patients across the lifespan, from infants to adults.
• Comfortable floating between Bronx and Brooklyn locations as required.
• Individual NYS Medicaid provider registration is mandatory.
• Proficiency in Spanish or other languages is preferred but not required.
• Strong commitment to serving underserved and vulnerable populations.
