Primary Care Physician Key Responsibilities & Duties

• Deliver comprehensive care for patients with acute and chronic conditions, ensuring optimal health outcomes.
• Lead care teams, including medical assistants and care coordinators, to provide seamless patient care.
• Engage with hospitalists and transitional care teams to ensure continuity of care for hospitalized patients.

Primary Care Physician Job Requirements

• Doctor of Medicine (MD) degree with active state medical licensure.
• Board certification in Internal Medicine, Family Medicine, or Geriatrics.
• Minimum of 1 year of experience, with 5 years preferred in a clinical setting.
• Proficiency in Microsoft Office Suite and electronic medical record systems.
• Basic Life Support (BLS) certification within 90 days of employment.
• Strong commitment to patient-centered care and value-based healthcare models.
• Ability to travel locally for training and occasional regional or national events.
